Course Request Change form Please use this form if you would like to request a change to a second semester course.  The deadline to request a change to a second semester (1/2 credit) course is December 1, 2025. 

The deadline to change a course request was June 15, 2025. Please review the guide below regarding schedule change requests. If you have an error in your schedule, please use this form to request a schedule change. 

Course Waiver/Level Change Form: Please use this for if you would like to request a different level course than what your teacher has recommended.  This form will also be used for any level changes in the same course by the end of the first quarter of the 26-27 school year. An example would be Honors to College Prep (CP).
